Job Summary
Conduct thorough assessments of residents' physical and mental health, develop individualized nursing care plans, and administer medications while monitoring vital signs and reporting changes promptly. Provide wound care, assist with activities of daily living, and document all care accurately in medical records. Educate residents and families on health conditions and treatments while ensuring compliance with regulatory standards and infection control protocols. Participate actively in care team meetings and quality improvement initiatives. Part-time evening shift (10:00pm-6:30am) with a $2 shift differential. Valid LPN/RN license and BLS certification required.
Required Qualifications
- Valid and current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of employment
- Completion of an accredited nursing program (LPN or RN)
- Basic Life Support (BLS) certification
- Strong understanding of nursing principles, medication administration, and patient care standards
Desired Qualifications
- Demonstrated experience in a long-term care or nursing home setting
- Certification in Geriatric Nursing or Long-Term Care Nursing
- Experience with electronic health records (EHR) systems
- Advanced certifications such as ACLS (Advanced Cardiovascular Life Support) or PALS (Pediatric Advanced Life Support)
- Proven ability to work effectively within interdisciplinary teams
- Additional training in wound care, dementia care, or palliative care
- Proficiency with electronic health records
- Specialized certifications
