Nurse, LPN/LVN PT-AM

Provide direct patient care as an LPN/LVN in assisted living, skilled nursing, and memory care settings; supervise and monitor nursing assistants to maintain high standards of care; participate in orientation and ongoing training for staff; communicate with the Nurse Manager/Director of Nursing regarding resident observations and concerns; observe and assess residents’ health and communicate significant changes; assist with procedures and care plans, including catheter placement and care, colostomy care, suction, dressings, and naso-gastric tubes; ensure accurate transcription and communication of physician orders for diets, therapies, medications, and other treatments; participate in shift handoffs to ensure continuity of care; uphold nursing standards and state/federal regulations while supporting a faith-based community.
