Nurse -LPN, ACT (25665)

Coordinate appropriate medical, dental, and mental health services for adult clients in the Assertive Community Treatment program, meeting them in their homes within Saratoga, Warren, and Washington counties. Support the RN and Psychiatric Nurse Practitioner by reviewing lab results, managing medication orders and prior authorizations, dispensing drugs, and overseeing client medication administration. Provide client and family education on treatments while documenting care in the electronic health record. Collaborate with the ACT team during daily meetings, communicate critical values to providers, and support admissions and discharges. Work onsite and in the community reporting to the Director of the ACT Program.
