Job Summary

Consult Care Management staff on medical issues including diabetes, medication, wound care, and acute or chronic illnesses. Participate in multidisciplinary team meetings and care conferences to recommend safe levels of care. Ensure individuals transitioning from institutional settings have long-term supports for safe community living. Review discharge paperwork and consult with inpatient, outpatient, and primary care providers. Provide ongoing trainings to CalAIM staff and complete required assessments for waived services programs. Liaise with Care Management to ensure clinical documentation meets best practice guidelines. Analyze utilization patterns to assist staff with health promotion interventions. Monitor service quality and contribute to Continuous Quality Improvement efforts. Attend team meetings and apply regulatory requirements of the Institute on Aging.

Required Qualifications

  • Registered Nurse (R.N.) license in California
  • Minimum two years working with disabled adults and/or older adults as a registered nurse
  • Experience providing trainings and education to a diverse group
  • Excellent verbal and written communication skills
  • Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks
  • Computer literacy

Desired Qualifications

  • Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders
  • Prior experience working with a health plan and/or in a SNF/Hospital setting
