Job Summary

Manage hospital and facility discharges for patients returning to the community or remaining in a facility setting. Provide backup support to the Intake Coordinator and perform community education, liaison, and outreach activities with referral sources to generate word-of-mouth referrals. Develop and maintain relationships with health partners and organizations, including Atlantic Health Services and St. Luke's, while sourcing new referrals and enhancing hospital connections through committee participation. Collaborate with discharge planners and clinical intake teams on hospice referrals, conducting General Inpatient Assessments, Evaluations, and admissions as needed. Submit activity reports and statistical analysis to the Administrator, manage data for outreach efforts, and participate in required ethics and palliative care meetings.

Required Qualifications

  • Knowledge of the dying process
  • Knowledge of policies and procedures of the hospice organization
  • Knowledge of regulations surrounding compliance with Medicare Conditions of Participation
  • Ability to function as a resource to patients and families during times of high stress and volatility
  • Ability to assess patients to determine appropriateness of participation in hospice
  • Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals
  • Excellent communication skills to effectively deliver information between referral source and the hospice clinical team
  • Consistently model respect for, and accountability to, the public, patients/families, and co-workers
  • Communication and actions reflect care, courtesy, honesty, and professionalism
  • Convey a professional and positive image and attitude regarding the hospice organization
  • Graduate of an NLN accredited school of nursing
  • Current licensure in New Jersey
  • RN required
  • 3-5 years working a healthcare environment with strong clinical skills
  • Exposure to hospice care

Desired Qualifications

  • BSN
  • Pennsylvania Nursing License
  • Prior experience in a liaison-type role
  • Prior hospice experience
