Job Summary
RN Case Manager performs complex nursing work providing for the assessment, care and integrated treatment of individuals residing at the SSLC. Exercises nursing judgment within the interdisciplinary team to ensure high-quality care, develops and implements nursing care plans, conducts monthly/quarterly/annual physical assessments, documents health care needs, reviews orders and MARs, provides hands-on nursing care, mentors new RNs, participates in QA and committees, educates team and families about care plans, and adheres to safety, infection control, and regulatory requirements. May participate in disaster/COOP activities and may telework per agency policy; active licensure and continuing education required.
Required Qualifications
- Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact
- At least one year experience as a practicing registered nurse is required
- Experience in assessment and documentation of health status is required
- Bachelor's Degree is preferred
- Previous experience working with Individuals with Intellectual and/or Developmental Disabilities (IDD) is preferred
- Fingerprint criminal background check, pre-employment drug screen, CANRS, Nurse Aide, Medication Aide and LEIE checks may be required
- All State Supported Living Center Employees are subject to Random Drug Testing
- Males between ages 18–25 must be registered with Selective Service
- Flexibility in work hours; may require overtime; weekends/holidays may be required
- Must be 18 years old or older for state-operated facility employment
