Job Summary
Provides basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis. Communicates with the interdisciplinary team about significant changes, documents assessments, participates in care planning, updates care plans, and implements nursing interventions including medication administration and IV care as appropriate. Delivers safe and timely care, responds to call lights, and supports resident and family education. Maintains SBARR communication with physicians and supports QAPI-driven problem resolution. Requires LPN license, valid CPR/AED, and IV certification (or willingness to obtain within six months).
Required Qualifications
- Licensed Practical Nurse (LPN) license in the state
- IV Certification required or willing to obtain within six months
- CPR/AED Certification required or willing to obtain within six months
- Ability to assess basic nursing care needs of geriatric residents
- Excellent communication, time management and organizational skills
- Critical thinking skills
- Documentation of nursing assessments and care plans
- Ability to participate in care planning and implement nursing interventions including medication administration and IV care as appropriate
