Job Summary
Prepare, monitor, and administer medication to residents at scheduled intervals, including assisting those with diabetes. Review Resident Service Plans, Communication Logs, and Medication Administration Records for accuracy while completing comprehensive assessments and behavioral screens. Create new care plans for residents returning from the hospital and coordinate transportation and medical appointments with the Resident Services Director. Perform regular audits on PRN, bulk, and controlled medications, notifying the Pharmacy of resident moves or hospital admissions. Maintain daily written and verbal communication with the Executive Director, file resident files, and ensure compliance with state regulations and drug-free workplace policies.
Required Qualifications
- current LPN license
- Familiar with gerontology and an overall view of health care delivery, including State regulations
- Must have an overall 'caring' attitude
- Excellent verbal and written communication, including listening skills
- Must have ability to supervise others
- Must be able to lift 75 pounds unassisted and 125 pounds per person in a two-person lift
- Will be regular in attendance, with the right attitude and attire
- Valid driver's license in the state of employment
