Nurse Coordinator, Supportive Medicine

Job Summary
Coordinate care for patients referred to the Supportive Medicine consult service, develop holistic care plans based on patient and family needs, and perform skilled nursing assessments. Anticipate and assist with pain and symptom management; provide family support, discharge planning, and education; participate in both formal and informal education for disciplines; serve as a preceptor/mentor and collaborate with interdisciplinary teams to ensure safe, personalized, and efficient care. Maintain documentation in required information systems, monitor patient progress, and adapt care plans over time while supporting staff development and departmental goals.
Required Qualifications
- Bachelor's or Master's degree in nursing
- Current license to practice Nursing in the State of Texas
- Current American Heart Association BLS/CPR within 30 days of hire
- Three (3) years of nursing experience
- Experience / Knowledge / Skills: effective oral and written communication skills
