Nurse Coordinator - Home Infusion

Assess patients with input from parents and families, then collaborate with the health care team to plan, implement, and evaluate care across the continuum. Determine patient needs and priorities, documenting special considerations on the Patient Teaching Record while educating families and evaluating teaching effectiveness. Maintain ongoing communication with perioperative and cath lab teams, participate in in-services and professional development, and coordinate data collection for research aimed at improving patient care. Take call for after-hours and weekend issues. Requires BLS certification, three years of nursing experience, and pediatric expertise.
