Job Summary
Oversee the daily operations of direct care services, including evaluation of patient care, review of client charts, monitoring TB tests and medication orders, stock management, and ensuring program continuity. Lead recruitment and training, provide clinical supervision of staff and interns, ensure high-quality care and KPI achievement, assist with discharge planning and follow-up services, and act as a liaison between clients, families, and community programs. Maintain strong communication, adhere to policies, and support quality and safety initiatives across Centerstone’s outpatient and residential services; ensure documentation and compliance with standards and regulations. Strong emphasis on supervision, program planning, crisis intervention, and collaboration within treatment teams.
Required Qualifications
- Education Level Diploma in Registered Nursing or Greater (Illinois RN license)
- Five years of successful program/service experience including supervisory or management duties, preferably in an outpatient setting
- Illinois Registered Nurse (RN) license or eligibility
