Job Summary
Senior Nurse Case Manager (Bilingual) responsible for care management for members with complex and chronic needs. Conducts assessments and develops, implements, coordinates, monitors and evaluates care plans to optimize health across the care continuum. Performs telephonic and on-site discharge planning, coordinates resources, negotiates related referrals/authorizations, and participates in interdepartmental and cross-brand workgroups. Requires a BA/BS in a health-related field, an active RN license, and a minimum of 5 years of clinical experience; bilingual capabilities noted in title. Hybrid work arrangement with in-office presence required four days per week; location considerations noted for commuting distance. Required Qualifications
- BA/BS in a health-related field
- RN license in applicable State
- Minimum of 5 years of clinical experience
- Preferred: Certification as a Case Manager
- Active, unrestricted DC RN licensure highly preferred
- Policy on hybrid/virtual work with in-office requirement at least weekly
