Nurse Case Manager

Lead and manage a care team of healthcare professionals, including nurses, social workers, therapists, and health coaches; actively engage with patients to help them understand the care model and disease management strategies; monitor and enhance care-team productivity; collaborate on primary care billing optimization; manage service recovery and conflict resolution; ensure right-time delivery of services to patients, families, facilities, and partners; oversee scheduling for case managers to maintain structured, effective provider timetables; promote effective two-way communication between staff and administration; provide training and supervision to ensure responsibilities, goals, protocols, and software usage are understood; participate in interviews and staff hiring; perform general administrative tasks and support other staff; leverage technology platforms to enhance patient care and productivity; participate in coaching calls and other duties as required by the company.
