Job Summary
Nurse Case Manager / RN Supports Coordinator works with elderly and disabled participants to assess needs, develops care plans using person-centered planning, coordinates Medicaid Waiver services, and monitors program eligibility. Performs comprehensive assessments as part of a multidisciplinary team, maintains documentation, advocates for participant access to benefits, and participates in quality assurance. Requires strong assessment, communication, and teamwork skills; must hold a current Michigan RN license.
Required Qualifications
- Must be a Registered Nurse.
- Must be a Registered Nurse (RN) with a current Michigan Nursing License, in good standing
- One year of experience working with the elderly and/or disabled is required.
