Job Summary
Lead and manage a care team of nurses, social workers, therapists, and health coaches within your assigned region. Actively engage with patients to explain care models, disease strategies, and management plans while monitoring team productivity to ensure efficient assessments and visits. Optimize primary care billing to reduce unnecessary spending and collaborate on conflict resolution and complaint management. Oversee scheduling processes, provide training on protocols and software systems, and participate in coaching calls and hiring interviews. Maintain consistent program reviews with leadership to ensure timely service delivery and promote two-way communication across the region. Adhere to normal work hours and company policies while supporting a culture of compliance and outstanding customer service.
