Job Summary
Nurse Case Manager (RN) in Hawaii providing palliative and holistic care within Hospice Hawaiʻi. Collaborates with the physician and Interdisciplinary Team to assess, plan, implement, and evaluate the plan of care for patients and their caregivers; delivers direct supervision of Hospice Aides; offers case management services aligned with care goals; adheres to regulatory standards; documents nursing and case management activities; maintains patient caseload and participates in on-call rotations (evenings/weekends/holidays); travels to patient homes; participates in Quality Improvement, Performance Improvement, and Compliance initiatives; engages in required in-service education and professional development; strong judgment, communication, multitasking, and teamwork are essential.
Required Qualifications
- Valid Registered Nurse License in the State of Hawaiʻi
- BSN preferred
- 1 year of experience as a staff nurse in an acute hospital
- Hospice and/or home health nursing experience preferred
- A legally compliant background check
- Drug screen & physical exam
- Valid Hawai‘i Driver’s License
- TB clearance
- CPR & BLS Certification
