Job Summary
Nurse Case Manager position with in-home and facility-based patient care in the Grand Strand area. Responsibilities include visiting patients in homes and facilities (ALF/ILF), reviewing medical records, enrolling patients in Care Management, initiating Plans of Care, capturing diagnoses, coordinating with providers, initiating telemedicine visits, and evaluating for home health, RPM devices, and other resources. Travel is daily to service areas including Murrell Inlet/Seabrook, Murrells Inlet/Myrtle Beach, Myrtle Beach, and Conway. Full-time, 8-hour shifts, Monday–Friday (8am–5pm). Must be a licensed nurse in good standing; driver with a reliable automobile is required. Qualifications emphasize strong communication, collaboration with interdisciplinary teams, and the ability to document and log Care Management activities.
