Job Summary
Nurse Case Manager position for a licensed Registered Nurse to service patients in the Grandstrand area, traveling to patients' homes and facilities to evaluate care needs and initiate telemedicine visits. Responsibilities include reviewing medical records, enrolling patients in Care Management, developing plans of care, capturing diagnoses, conducting AWVs and cognitive/SDoH/ACP assessments, coordinating care with care teams, reconciling medications, providing post-discharge education, evaluating for adaptive equipment and DME, and ensuring up-to-date care management documentation. Requires reliable transportation and travel between Conway, Murrells Inlet, and surrounding areas. Full-time, 8-hour shifts, Monday–Friday, 8am–5pm, with competitive compensation, benefits, 401K, vehicle allowance, PTO, and growth opportunities.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- A minimum of three years clinical experience preferred
- Experience in community settings preferred
- Strong written and verbal skills
- Ability to manage and demonstrate effective leadership and collaboration with interdisciplinary care teams
- Must be able to travel between offices, facilities, etc.
- Must be a licensed driver with automobile insured and in good working order
