Job Summary
Nurse Case Manager position for a licensed RN to service patients across the Marietta area, traveling to homes and long-term care facilities to evaluate patients and initiate telemedicine visits. Role includes coordinating with care teams, reviewing medical records, enrolling patients in Care Management, and developing Plans of Care. Requires a valid driver’s license and reliable transportation; 8-hour shifts, Monday–Friday, in-office with travel to service sites within designated area. Required Qualifications
- Must be a licensed nurse.
- Registered nurse preferred.
- License must be in good standing with appropriate board/issuer.
- Must be able to travel to patients' homes and facilities (home visits).
- Able to operate a vehicle and have reliable transportation.
