NURSE CASE MANAGER PRN/Part-time

Job Summary
Conduct comprehensive patient assessments, develop individualized care plans, and deliver hands-on skilled nursing services including wound care, medication administration, and IV therapy in the home. Manage a caseload of home health patients by coordinating with physicians, therapists, aides, and family members while supervising field staff and ensuring compliance with Medicare/Medicaid guidelines and OASIS documentation standards. Provide clinical oversight through supervisory visits, case conferences, and performance improvement initiatives to maintain high-quality, patient-centered care. Requires an active Indiana RN license, 1–2 years of experience, valid driver's license, and current CPR certification.
Required Qualifications
- Active Registered Nurse (RN) license in the State of Indiana (or applicable state)
- Minimum 1–2 years of nursing experience
- Strong clinical assessment and critical thinking skills
- Ability to independently manage time, priorities, and a patient caseload
- Excellent communication and interpersonal skills
- Valid driver's license, reliable transportation, and current auto insurance
- Experience with OASIS documentation and EMR systems
- Knowledge of Medicare, Medicaid, and state home health regulations
- Current CPR certification
