Job Summary
Nurse Case Manager with active RN license in Oregon, to provide in-person hospice/home health care. Duties include assessing patients, developing and implementing nursing care plans, administering nursing care, coordinating with physicians and the care team, supervising LPNs and home health aides, managing a caseload, maintaining detailed records, coordinating diagnostic tests, ensuring regulatory compliance, and contributing to ongoing education and quality of care. Strong organizational and communication skills, and ability to work without direct supervision in a high-paced environment are required.
