Nurse Case Manager - Per Diem

Coordinate and support healthcare within the facility while managing referrals for services outside the clinic. Provide outreach and enrollment services to meet eligibility requirements, then modify patient treatment plans based on individual responses and conditions. Prepare patients and families for discharge, define care goals through education, and partner with service providers to create comprehensive community service plans. Communicate with third-party payers to obtain necessary authorizations and review challenging cases with medical directors. Maintain accurate reports and records while ensuring coordination of community and family support.
