Job Summary
Licensed RN or LPN Nurse Case Manager for the PACT team to deliver nursing interventions, coordinate medical care, manage medications, and develop Comprehensive Assessments and IAPs for adults with severe mental illness. Responsibilities include primary case management for 3-6 consumers, health education, transportation to work/sites, and rotating on-call/evening shifts. Requires an associate degree in nursing and an active RN/LPN license; Massachusetts driver’s license is noted. Schedule Monday–Friday 8am–4pm with rotating 12pm–8pm shift; one weekend per month; rotating holidays and on-call. Salary range $58,073.60–$77,680 USD annually. Bilingual candidates may receive a 6% salary increase.
Required Qualifications
- associates degree in nursing
- LPN or RN nursing license
- Experience working with adults with mental illness
- knowledge of psychiatric and medical medications
- Valid Massachusetts drivers license
