Job Summary
Nurse Case Manager for the PACT program responsible for acting as primary case manager for 3–6 consumers, providing nursing interventions, coordinating medical care and health education, managing medications, and maintaining comprehensive assessments. Role includes transporting clients to work sites/appointments, participating in rotating on-call coverage and evenings to ensure 24/7 support. Requires RN or LPN license, associate degree in nursing, and MA driver's license; experience with adults with mental illness and medications. Schedule: Mon-Fri 8am-4pm with rotating extended hours including one weekend per month and rotating holidays; salary in the $58k–$77k range. Required Qualifications
- Licensed RN or LPN
- Associates degree in nursing
- Valid Massachusetts driver's license
