Job Summary
Nurse Case Manager on Eliot’s PACT team: provide primary case management to 3-6 adults with severe mental illness, perform nursing interventions, coordinate medical care with providers, deliver health education, assist with medication management, develop and update Comprehensive Assessments and Individualized Adaptive Plans (IAPs), and transport clients to work sites and appointments. Requires an associates degree in nursing and a valid nursing license (LPN or RN) and a valid Massachusetts driver’s license. scheduling Monday–Friday with rotating 12pm–8pm shift, one weekend per month, on-call rotating duties; bilingual candidates may receive a salary increase. Annual salary range: $58,073.60–$77,680 USD.
