Job Summary
Nurse Case Manager (NCM) providing clinical case management, health promotion, and coordination of care for RC Soldiers across TRICARE, VA, and civilian providers. Works as part of the USARC Psychological Health Program regional team to coordinate care across readiness divisions, conduct assessments, develop and monitor individualized care plans, coordinate referrals, provide psychoeducation at events, and liaise with commanders and health systems. Requires RN licensure (ADN minimum) and active nursing certifications, with potential licensure in additional states for telephonic services. Role includes regional travel and flexible shift requirements; medical documentation and compliance responsibilities with MSR reporting, and maintaining licensure and professional certifications throughout contract performance.
Required Qualifications
- Associate Degree in Nursing (ADN) minimum
- Current, valid, unrestricted RN license in state of performance; licensure in additional states where telephonic services are provided
- Active nursing-related professional certification (e.g., CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, CMC)
- Minimum 2 years RN case management experience within last 3 years
- Experience in complex organizational environments; familiarity with military culture, TRICARE, VA health systems preferred
- Clear LEIE, TRICARE sanction query, and FBI/state background check required
- US citizenship or authorized work status; ability to obtain CAC (Tier 1 investigation)
- Prior experience with military Servicemembers/veterans or RC/National Guard populations preferred
Desired Qualifications
- RN License
- Case Manager Certification (CCM, CDMS, CRC, COHN/COHN-S, ACCC, CMC, RN-NCM, ANCC)
- 2+ years RN case management experience
- ACEN- or CCNE-accredited nursing program graduate
- Ability to obtain licensure in additional states for telephonic services
- U.S. citizenship or authorized work status
- CAC (Tier 1) investigation readiness
- Experience with TRICARE and VA health systems (preferred)
- Military service member/veteran population experience (preferred)
