Job Summary
Nurse Case Manager (NCM) supporting the USARC Psychological Health Program. Responsible for clinical case management, health promotion, and coordination of care across TRICARE and VA for RC Soldiers; liaises with commanders, health care systems, and regionally-based PH programs; conducts assessments, develops and monitors care plans, coordinates referrals, provides psychoeducation at RC events, and maintains documentation in compliance with regulations. Requires RN licensure in performance state, active nursing certifications in approved specialties, and at least 2 years of RN case management experience; travel regionally may be required; position contingent upon contract award.
Required Qualifications
- Associate Degree of Nursing (ADN) minimum
- Current, valid, unrestricted Registered Nurse (RN) license in the state of performance; must obtain licensure in any additional state where telephonic services are provided
- Active certification in one of the following: Certified Case Manager (CCM – CCMC); Certified Disability Management Specialist (CDMS); Certified Rehabilitation Registered Nurse (CRRN); Certified Occupational Health Nurse (COHN or COHN-S); Advanced Certification in Continuity of Care (ACCC); Certified Rehabilitation Counselor (CRC); Nurse Case Manager (RN-NCM – ANCC); or Care Manager Certified (CMC)
- Minimum 2 years of experience as a registered nurse case manager within the last 3 years
- Experience in complex organizational environments; familiarity with military culture, TRICARE, and VA health systems strongly preferred
- Clear LEIE, TRICARE sanction query, and FBI/state background check required
- U.S. citizenship or authorized work status; ability to obtain CAC (Tier 1 investigation)
