Job Summary
Nurse Case Manager to support the USARC Psychological Health Program (PHP) for RC Soldiers across TRICARE/VA networks; provide clinical case management, health promotion, and consultation; coordinate referrals and follow-ups; liaison between Soldiers, commanders, and healthcare systems; conduct assessments, plan and coordinate regional case management meetings, and maintain compliant case records. Requires ADN minimum, RN licensure in performance state, active certifications in CCM/CDMS/CRRN/COHN-COHN-S/ACCC/CRC/RN-NCM/CMC, and a minimum of 2 years RN case management experience; travel and varied shift requirements; contingent upon contract award.
Required Qualifications
- Associate Degree of Nursing (ADN) minimum
- Current, valid, unrestricted RN license in state of performance
- Active certification in CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC
- Minimum 2 years of RN case management experience within last 3 years
- Familiarity with TRICARE and VA health systems preferred
- Clear LEIE, TRICARE sanction query, and FBI/state background check required
- U.S. citizenship or authorized work status; ability to obtain CAC (Tier 1 investigation)
- Experience in managing complex organizational environments
- Preferred experience with military servicemembers, veterans, RC/National Guard
- Knowledge of TRICARE, VA case management processes, and SRP
