Nurse Case Manager - Minneapolis / St. Paul, MN

Nurse Case Manager position covering the Minneapolis / St. Paul area. Responsible for consultation, development, and implementation of rehabilitation plans for injured workers/consumers, working independently to support return-to-work goals. Collaborates with injured workers, claims examiners, employers, attorneys, and medical providers to ensure timely, accurate documentation and progress toward rehabilitation. Responsibilities include accepting referral cases, counseling and motivating consumers, maintaining confidential communication with all parties, producing timely written reports, ensuring rehabilitation plan implementation, and supporting sales/marketing efforts to grow caseload. Requires RN license in MN, plus QRC qualifications or internship, and 2 years of full-time equivalent direct clinical experience. Strong time management, communication, problem-solving, and proficiency with case management software are essential.
