Job Summary
Coordinate the overall interdisciplinary plan of care for patients from admission to discharge, monitoring care and acting as a liaison between patients, families, healthcare personnel, and insurers. Evaluate patient needs and available resources to recommend and facilitate discharge plans that encourage compliance with medical advice. Identify care issues and suggest revisions to clinical pathways to improve quality, while conducting utilization reviews as needed. Manage the care continuum to ensure efficient patient movement through the hospital system. Work 18 weekly hours on a PRN basis.
Required Qualifications
- Bachelor's degree in Nursing from an accredited school of nursing
- three years work experience as a Registered Nurse
- Licensure as a registered nurse by the state of South Carolina or compact state
- Current Basic Life Support (BLS) or ACLS certification
- Ability to lift and carry 50 lbs. unassisted
- Ability to lift/lower objects 50 lbs. from/to floor from/to 36 inches unassisted
- Ability to lift from 36" to overhead 25 lbs.
- Ability to exert up to 50 lbs. of force
- Ability to maintain 20/40 vision, corrected, in one eye or with both eyes
- Ability to see and recognize objects close at hand or at a distance
- Ability to match or discriminate between colors
- Ability to determine distance/relationship between objects; depth perception
- Good peripheral vision capabilities
- Ability to maintain hearing acuity, with correction
- Ability to maintain tactile sensory functions
- Ability to maintain good olfactory sensory function
- Ability to be qualified physically for respirator use, initially and as required
- Ability to work rotating shifts
- Ability to work overtime as required
- Ability to work in a latex safe environment
- Ability to maintain 20/40 vision, corrected, in one eye or with both eyes
Desired Qualifications
- Prior leadership experience
- One year of case management and/or utilization review work experience
- Case Management certification
