Job Summary
Nurse Case Manager I provides care management for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health across the care continuum. Responsibilities include conducting assessments to identify needs, developing care plans, facilitating authorizations/referrals within benefits or via extra-contractual arrangements, coordinating internal and external resources, and monitoring effectiveness of the care management plan. Requires BA/BS in a health-related field, current RN license, and a minimum of 3 years of clinical experience (multi-state licensure required if serving multiple states). Preferred: Case Manager certification. Hybrid/virtual work arrangement with in-person onboarding and training; location specified as Latham, NY. Vaccination for COVID-19 and Influenza may be required for certain patient/member-facing roles. Required Qualifications
