Job Summary
Assess, develop, and implement care plans for members with complex chronic needs by conducting on-site visits at hospitals, homes, or clinics. Coordinate internal and external resources to facilitate referrals and decrease hospital or ER readmissions. Monitor care plan effectiveness, modify plans as necessary, and interface with Medical Directors and Physician Advisors. Perform duties telephonically or in the field two to three times weekly based on business needs.
Required Qualifications
- BA/BS in a health-related field
- minimum of 3 years of clinical experience
- Current, unrestricted RN license within the state of Texas
- Multi-state licensure if providing services in multiple states
- Reside within Angelina, Walker (Huntsville), Tom Green (San Angelo), and Taylor counties in Texas
- Available for field work 2x-3x a week
- COVID-19 vaccination
- Influenza vaccination
Desired Qualifications
- Certification as a Case Manager
- Experience with Microsoft TEAMs, Outlook, Word, and Excel
- Experience with Home Health completing field visits
- Experience with Medicaid
- Positive, strong interpersonal and communication skills
