Nurse Case Manager - Hartford, CT

Field Nurse Case Manager covering Hartford and surrounding CT/MA communities (including New Britain, Bristol, Manchester, Glastonbury, and Springfield, MA). Field/home-based role with travel to doctors’ offices, hospitals, and other locations. Responsible for medical case management of work-related injuries: assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. Collaborates with insurance carriers, medical care providers, attorneys, employers, and employees; monitors progress and reports results. Must be willing to learn and take catastrophic cases. Requires RN licensure in CT and MA, strong organizational and communication skills, autonomous work style, patient education skills, and a valid driver’s license. Must be prepared for travel within the assigned coverage area; travel expectations include up to 2 hours one-way in the field.
