Job Summary
Field Nurse Case Manager to cover Hartford and surrounding communities (including New Britain, Bristol, Manchester, Glastonbury, and Springfield, MA). This field/home-based role involves travel to doctors’ offices, hospitals, and various locations, with up to 2 hours one-way travel. Responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation, and collaboration with insurance carriers, medical providers, attorneys, employers, and employees. Must be willing to learn and take catastrophic cases; requires valid driver’s license and good driving history; autonomous work style with strong organization, communication, and technology skills.
Required Qualifications
- Current, unencumbered RN license in CT
- Current, unencumbered RN license in MA (if not held, start process within 30 days)
- Experience in case management of workers' compensation cases (preferred)
- Willingness to learn and take catastrophic cases
- Certifications recommended (CCM, CRRN, CDMS, CRC) with REQUIRED within two years of hire
