Job Summary
Field Nurse Case Manager role based in Hartford area with travel to surrounding CT locales (Manchester, Glastonbury, Bristol, New Britain) and Springfield, MA. Travel up to 2 hours one-way to doctors’ offices, hospitals, and other locations. Responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. Collaborates with insurance carriers, medical providers, attorneys, employers, and employees; monitors progress and reports results to employers and carriers. Requires autonomous work style, strong organization, and patient education skills; must maintain RN licensure in CT and MA and pursue advanced certifications within two years. Driver’s license and clean driving history required.
Required Qualifications
- Current, unencumbered Registered Nurse (RN) license in CT
- Current, unencumbered Registered Nurse (RN) license in MA
- Experience in case management of workers' comp cases (preferred)
- Willingness to learn and take catastrophic cases
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; REQUIRED within two years of hire
- Valid Driver’s License, and good driving history
