Job Summary
Nurse Case Manager – field/home-based role covering Hartford and nearby communities (CT) and parts of MA, with travel to doctors’ offices, hospitals, and other locations. Responsible for medical case management of work-related injuries: assessment, planning, coordination, implementation, and evaluation. Requires CT RN license and MA RN license; CCM/CRRN/CDMS/CRC certifications preferred within two years of hire. Travel up to two hours one-way (4 hours round trip). Independent work style, strong organizational and communication skills; driving and adherence to timelines essential. Hybrid/field-based with travel; supports collaboration with insurers, medical providers, employers, and employees.
Required Qualifications
- Current, unencumbered Registered Nurse (RN) license in CT
- Current, unencumbered Registered Nurse (RN) license in MA
- REQUIRED within two years of hire: CCM, CRRN, CDMS, CRC (preferred)
- Experience in case management of workers' comp cases (preferred)
- Skills: organization, timeline adherence, PC and technology, communication (written, verbal, interpersonal)
- Valid Driver’s License and clean driving history
