Job Summary
Nurse Case Manager provides monitoring and oversight of participants’ medical care in the Enhanced Step-Down program, including nursing assessments, care coordination with partner hospitals, intake/discharge activities, development and monitoring of the Plan of Nursing Services (PONS), review of medical records, scheduling and accompanying medical appointments, and maintenance of participant confidentiality. Requires NYS RN licensure and at least one year of experience in mental health or I/DD; involve CTI/TRS collaboration, on-call duties, and ensuring regulatory compliance while supporting a small caseload of participants.
Required Qualifications
- NYS Registered Nurse licensure
- One year of experience preferred working with individuals with mental health and/or I/DD diagnoses
- Valid NYS Driver License as per agency vehicle policy
- Transportation to meet program needs
- Ability to perform repetitive motion
- Ability to lift 75lbs
- Flexible schedule for emergencies
- Maintain NYS RN recertification requirements
