Job Summary
Nurse Case Manager provides medical case management, screening, education, counseling, and medication management for residents in Cunningham Children's Home's Health Services Team. Responsibilities include coordinating with community health care providers, participating in a rotating on-call schedule (primarily phone consultations), and ensuring accessibility without requirement for on-site presence. Requires RN licensure in Illinois and either a BSN or ASN; offers flexible scheduling and comprehensive benefits in a non-profit residential setting focused on youth care, with opportunities to engage in psychiatric care support for children/adolescents.
Required Qualifications
- Bachelor of Science in Nursing or Associate of Science in Nursing
- Current licensure in the State of Illinois as a Registered Nurse
- Current State of Illinois Driver's License
- Graduation from an accredited nursing program
Desired Qualifications
- RN licensure in Illinois
