Job Summary
Nurse Case Manager provides medical case management, screening, education, counseling, medication management, and networking with community health care providers for youth in residential services; participates in a rotating on-call schedule, primarily providing phone consultations; requires RN licensure in Illinois with BSN/ASN acceptable; experience with psychiatric care and children/adolescents preferred; offers flexible scheduling and a comprehensive benefits package including health insurance, retirement plan, and relocation assistance; serves as part of the Health Services Team at Cunningham Children's Home in Urbana, IL.
Required Qualifications
- Bachelor of Science Nursing or Associate of Science in Nursing
- Graduation from an accredited nursing program
- Current licensure in the State of Illinois as a Registered Nurse
- Current State of Illinois Driver's License required
