Job Summary
Nurse Case Manager responsible for medical case management, screening, education, counseling, and medication management for youth in residential services; build networks with community health care providers; participate in a rotating on-call schedule (primarily phone consultations) to ensure accessibility without mandatory on-site presence; requires an Illinois-licensed RN with a BSN or ASN and active driver's license. Required Qualifications
- Bachelor of Science in Nursing or Associate of Science in Nursing
- Current licensure in the State of Illinois as a Registered Nurse
- Current State of Illinois Driver's License
- Experience with psychiatric care and children/adolescents preferred
