Provide medical case management, screening, education, counseling, and medication management for residents in Cunningham Children's Home; build and maintain connections with community health care providers; participate in a rotating on-call schedule, primarily via phone consultations, with on-site presence as needed within the Health Services Team.
Required Qualifications
- Bachelor of Science in Nursing or Associate of Science in Nursing
- Graduation from an accredited nursing program
- Current licensure in the State of Illinois as a Registered Nurse
- Current State of Illinois Driver's License
