Job Summary
Nurse Case Manager responsible for medical case management, screening, education, counseling, medication management, and networking with community health care providers; participates in a rotating on-call schedule with phone consultations, primarily no on-site presence. Requires BSN or ASN and current Illinois RN license plus a valid IL driver’s license. Experience with psychiatric care and children/adolescents preferred. Offers flexible scheduling and a benefits package including health, dental, vision, life insurance, 401(k) with company match, relocation assistance, and other programs. Located in Urbana, IL, within Cunningham Children’s Home Health Services Team.
Required Qualifications
- Bachelor of Science in Nursing or Associate of Science in Nursing
- Current licensure in the State of Illinois as a Registered Nurse
- Current State of Illinois Driver's License
- Experience with psychiatric care and children/adolescents is preferred
