Job Summary
Nurse Case Manager position in Bluffton/Okatie area requiring visiting patients across facilities, homes, clinics, and via telemedicine in a primary care setting. Role entails reviewing medical records, enrolling eligible patients in Care Management, initiating and updating a Care Plan of Care, coordinating with interdisciplinary care teams and providers, and ensuring documentation of care management activities. Must hold a nursing license (RN preferred) in good standing, with travel between offices daily, and a schedule of full-time, 8-hour shifts Monday–Friday (8am–5pm). Strong communication, coordination, and patient-focused care are essential, along with the ability to work with care teams, facilities, and families to ensure quality healthcare delivery.
Required Qualifications
- Must be a licensed nurse
- Registered nurse preferred
- License must be in good standing with appropriate board/issuer
- Travel between offices, must be a licensed driver with automobile in good working order
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
