Job Summary
Registered Nurse to service patients throughout the Hilton Head area as a Nurse Case Manager. The role involves visiting patients, coordinating with appropriate providers, initiating telemedicine visits within clinic settings, evaluating patients, and developing care management plans as eligible. Responsibilities include receiving medical records, enrolling patients in Care Management, coordinating with the care team, documenting activities, and ensuring high-quality, compliant care across facilities and virtual visits. Requires reliable transportation for daily travel and is a full-time position with 8-hour shifts (8am-5pm, Monday-Friday).
Required Qualifications
- Must be a licensed nurse
- Registered nurse preferred
- License must be in good standing with appropriate board/issuer
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Strong written and verbal skills
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
