Job Summary
Ensure CFHP members regain optimum health or improved functional capacity by coordinating access to necessary health care services efficiently. Assist with case management for members to achieve improved functional capacity while maintaining Monday through Friday 8:30 a.m. to 5:00 p.m. availability. Required Qualifications
- Three years acute care experience or managed care experience
- Current RN Texas License
