Nurse Case Manager

Develop and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams. Act as the primary advocate for patients and families, facilitating access to medical providers and community resources. Provide essential health education to improve chronic condition management and conduct assessments to identify unmet health and social needs. Ensure medication adherence and follow-up appointments through ongoing support while maintaining accurate documentation and communicating effectively with the care team. Work Monday through Friday on a stable day shift at Community Physicians, a multi-specialty group focused on geriatric care and post-acute settings.
