Job Summary
Nurse Case Manager provides patient and caregiver support, coordinates with health care providers, and manages patient access to therapies through program services. Role includes liaison between patients, families, and providers; documentation in electronic patient records; adherence monitoring and KPI-driven case management; collaboration with reimbursement and medical teams to secure funding and explore options; on-call duties and staff training responsibilities. Requires a health-care diploma and active nursing license (RN/RPN/LPN) with 2–5 years of relevant experience; fluency in French and/or English may be required depending on location. Required Qualifications
