Nurse Case Manager

Nurse Case Manager role supporting patient and provider access through Patient Support services and disease/therapy information. Serve as liaison between patients, families and health care providers, perform patient assessments, address inquiries about program services, support patient adherence, coordinate with health care providers for clinical questions beyond program materials, document interactions in electronic patient records, conduct ongoing file reviews, follow case-management protocols (data collection, patient follow-up, therapy disruption monitoring, adverse event reporting), collaborate with reimbursement specialists to secure funding and explore alternatives, ensure coordinated patient services according to KPIs, participate in specialized training, attend pharmaceutical meetings and engage with patient associations as needed, coordinate logistics with health care providers, pharmacies, and infusion/injection clinics, train new Nurse Case Managers, report adverse events, and may be on standby/on-call.
