Job Summary
Develop, implement, and update individualized plans of care based on assessed needs, client/family goals, and provider direction. Identify clinical risks and changing conditions; initiate timely interventions and escalations to providers, families, or emergency services when indicated. Collaborate with clients and families to set realistic goals and ensure day-to-day care supports those objectives. Monitor clinical outcomes, identify quality improvement opportunities, and provide field-based clinical oversight to ensure safe, consistent care delivery. Educate and coach Care Professionals on client-specific needs, safety, infection prevention, and documentation expectations. Communicate care plan updates and status changes to Care Coordinators and assigned professionals to support safe staffing and continuity. This full-time, salaried role begins at $120,000 annually and requires an active RN license, two years of nursing experience, and a valid driver's license.
Required Qualifications
- Graduate of an accredited nursing program
- Current, active, unrestricted Registered Nurse (RN) license in the state(s) of practice or ability to obtain per company hiring plan
- Minimum 2 years nursing experience
- Valid driver's license
- Reliable transportation
- Ability to travel locally with short notice
