Job Summary
Coordinate care across the continuum for assigned service lines by facilitating interdisciplinary planning, monitoring clinical processes against evidence-based guidelines, and leading discharge planning from admission. Conduct utilization review to clarify diagnoses, negotiate payer benefits, and identify high-risk patients for resource collaboration. Drive quality improvement initiatives to reduce length of stay and resource consumption while compiling aggregate variance data for leadership. Serve as a clinician, consultant, and educator for the healthcare team, community, and payers to optimize clinical and financial outcomes.
Required Qualifications
- Registered nurse
- Current license in NY State
- Minimum of three years clinical experience in an assigned service
- Demonstrates effective communication, facilitation, and organizational skills
- Assertive and creative in problem solving, critical thinking skills, systems planning and patient care management
- Self-directed with the ability to adapt in a changing environment
- Basic knowledge of computer systems with skills applicable to utilization review process
Desired Qualifications
- Bachelor's degree
- Acute care nursing and case management experience
- Recent experience in case management, utilization management and/or discharge planning/home care in a high volume, acute care hospital
- PRI and Case Management certification
